Scribe360, a product of Clear Vision 360, is a clinical AI scribe platform used by healthcare practices to document patient encounters and (optionally) deliver telehealth visit links and appointment reminders by SMS. Consent for SMS communications is collected by the practice during patient intake using a TCPA-compliant signed consent form (paper or digital). Practices may use their own intake form or a ready-to-print template Scribe360 provides. Before any SMS is sent, practice staff explicitly attest in the Scribe360 app that signed consent is on file for the patient — the timestamp, attesting user, and IP are recorded immutably for audit. Patients may reply STOP at any time to permanently opt out, with automatic confirmation. 3. Opt-In Flow
Consent is collected by the practice (the message sender) during patient intake, not by Scribe360 directly. The 5-step flow:
- Patient signs an intake consent form — paper or digital, on the practice's own intake form or the optional printable template we provide. Affirmative action only — no pre-checked boxes.
- Practice retains the signed consent record for at least four (4) years (TCPA records retention).
- Practice staff attests in Scribe360 by checking the SMS consent box on the patient record. The platform stamps timestamp + attesting user + IP into an immutable audit log.
- Scribe360 sends SMS only to patients with a current attestation on file. Without it, the platform refuses to send.
- Patient can reply STOP at any time. Opt-out is permanent and immediate; the platform clears the attestation. Reply HELP returns sender info and contact options.

"I authorize {Practice Name} to send me text messages relating to my care, including telehealth visit links, appointment reminders, and related communications. Standard message and data rates may apply. Message frequency varies based on my appointments. I understand SMS messages are not encrypted and are not protected by HIPAA in transit, and I accept this risk for the convenience of receiving care communications by text. I understand I can opt out at any time by replying STOP, or reply HELP for help. For minors, the parent or guardian providing this consent confirms their authority to do so."
4. Opt-Out Mechanism
- STOP keyword — patient replies STOP to any message. Scribe360 immediately and permanently clears the patient's SMS consent attestation; no further messages are sent until the patient re-subscribes via a new signed consent form.
- STOP confirmation — automatic single reply confirming unsubscribe (TCPA-required behavior).
- HELP keyword — automatic reply with sender identification and contact email.
- Email — patients may also email support@cv360.solutions to request unsubscribe.
